The Cambridge North Dumfries Ontario Health Team (CND OHT) is a Ministry of Health-funded collaborative of over 30 health and social service provider agencies from Cambridge, North Dumfries and the Waterloo Region. Ontario Health Teams increase the connectedness local services, making it easier for patients to navigate the system and transition between providers. The CND OHT envisions a community where people access seamless and integrated services to ensure the health and wellbeing of our diverse populations.

A: | When we chose to take on the task of redesigning our website, we reached out to our Members for samples and recommendations. One of our OHT members is the eHealth Centre of Excellence, and we were particularly impressed with the modern and clean look of their website. A quick connection with their team pointed us to REM and we are thrilled to have worked with them.
